--- WAIVER & RELEASE OF LIABILITY ---

I personally and as representative on behalf of the group, individual or organization identified on the preceding page, and in consideration of being permitted to enroll and participate in this event, acknowledge the following: that participation in this event is voluntary, and involves physical activity. I agree as to myself and all those I represent in making this application, for my/our/their heirs, personal representatives or assigns, to hereby assume the risk of any injury from any cause whatsoever, including the negligence of the City of DeLand, Rotary International, the local Rotary Clubs and their Parade Committee, Corporate Sponsors of this Event (collectively, the Event Organizers); their officials, officers, volunteers, agents, and employees, and to give up, waive, discharge and release the Event Organizers of this event from any and all claims for injuries, including death, which I or anyone in my group may or could sustain, and on behalf of myself and those I represent agree to hold the Event Organizers harmless, and covenant not to sue the Event Organizers should property loss, injury, or death occur before, during or as a result of participation in this event. By signing below, I acknowledge that I have read this assumption of risk, waiver and release of liability agreement, fully understand its terms, have secured the permission and authorization of those I represent to sign as their representative, and further assure the officials, organizations and others listed above that the group/individual/organization I represent understand(s) that I/we/they am(are) giving up substantial rights, including but not limited to the right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend personally and on behalf of those I represent for this to be a complete and unconditional release of all liability.

My group/individual/organization further authorizes the organizers of this event to reproduce, use and utilize all photographs, video and audio recordings in which I/we/the group may appear which are made during the event.

My group/individual/organization understands that this Event is intended to provide entertainment for the entire community, and for all age levels, and accordingly further agrees to adhere to and abide by all rules, guidelines, codes of conduct, instructions and directions of the Event Organizers, whether written or oral. Written guidelines may be reviewed at: delandchristmasparade.org. Additional guidelines may be developed and enforced by the Event Organizers through and during the Event to further provide for the safety of participants and spectators and to assure the "family" atmosphere and quality of the Event. The Event Organizers reserve the right to refuse entry and participation.

All forms and payments must be RECEIVED by November 8 or until registration is full, whichever comes first.

This parade is a fun community event; please keep to a positive expression on your entry. The organizers reserve the right to refuse entry and participation.
